We want to make being a new chiropractic and/or massage patient as easy as possible for you. Our new patient form(s) can be completed in person or electronically via an email link. You will be provided access to an individual portal account for ease of communication and documentation. We do recommend completing the forms prior to coming in for your first appointment for optimal use of your time. We also ask that you bring a photo ID and any applicable insurance.  Please feel free to reach out with any questions.

consultation

One of our skilled doctors will meet with you to review your health history and reason for care. You will have the opportunity to have your questions answered, express concerns, and discuss your goals. It is important to us that you understand the consultation portion is to establish what we can do for you. The consultation is no charge to you. If you are unsure if we can help, do not hesitate to come in and discuss it with our doctors.

examination

If you decide to move forward with care, an individualized exam will be performed by our team to determine the most appropriate course of care based on your condition(s). On-site x-rays are available for diagnostic purposes and may not be required. However; if you would like x-rays to be taken, we would be happy to do so! Your care plan will be established and never forced - we leave it up to you as we offer recommendations and guidance. Periodically throughout your care plan, you will be re-evaluated (follow-up exams) to track progress.

treatment

Your initial (New Patient) visit time can vary based on individual conditions and complexity, ranging from 30 minutes to 90 minutes. Everything we do is explained beforehand and you will decide what you are comfortable with and choose from different approaches we have for your treatment.

Very convenient hours! Great staff and Dr Ouillette may be the best chiro at adjustments out there. Not to mention everything else used during appointments. A mix between PT and Chiro.
